Making the numbers mean something since the first pitch

Out of 1,000 professional pitchers who have the operation, 791 pitch again. Only 735 get their level back. Same surgery, same pitchers, two different finish lines — and only one of them makes the headline.

The question: everyone says Tommy John surgery has a 90% success rate. Ninety percent of pitchers successfully did what?

The answer depends entirely on where you draw the line. A pitcher throws again. A pitcher returns to college baseball. A pitcher gets his old velocity back. All four might be called a success. They are not the same thing.

Here is what is surprising. The famous number is not wrong. The question it answers is incomplete. And the gap has grown into something worse than confusion — a belief among some players and parents that the surgery makes you better, and that a torn ligament is a door to an upgrade.

What you get: one extra question that changes almost any percentage you will ever be handed.

Same operation. Same thousand pitchers.

We start with counts rather than rates, because rates hide the people.

A 2025 study followed 2,281 major and minor league pitchers through UCL reconstruction. Scale it to a thousand and you can see the whole thing at once.

Now ask your question again, and watch the number move. How many pitched in a game again? 791. How many got back to the level they left? 735. How many held that level for two more seasons without another elbow surgery? Nobody has pooled that figure, which is itself worth knowing.

Same operation; same thousand men; three different statistics.

One pitch can make you a “return”

Picture three pitchers coming back from the same surgery. Watch what our definitions do to them.

The first returns to Division I, throws two full seasons, gets his velocity back and is drafted. The second returns, but a level down, with fewer innings and less on the fastball. The third gets into a game, pitches briefly, and his elbow goes again.

Did all three successfully return? Under most published rules, yes. All three are in the numerator.

That is why studies now separate return to play from return to the same level. The first asks whether he played again; the second asks what a pitcher’s family actually wants to know. Is he the pitcher he was?

We quietly turn “most pitchers pitch again” into “most pitchers come all the way back.” Those need different evidence.

The Superman story, and why it is backwards

Here is the belief this piece is really aimed at, and you have heard it said out loud.

That a pitcher comes back from Tommy John better than he was. Harder. Stronger. Some young players now push for the operation on that basis, as though a torn ligament were a door to an upgrade.

We can see where the story came from. A pitcher spends a year rebuilding his arm, his legs and his mechanics with more coaching attention than he has ever had. Some come back throwing hard. Cause and effect get swapped, and the year of work gets credited to the ligament.

The measured picture points the other way. Most studies find pitchers throwing fewer innings after the surgery than before it, and several find lower fastball velocity. No study shows the new ligament outperforming a healthy one, because that is not a thing a graft does.

Let us put it plainly, the way a surgeon would. You are not being given a better elbow. You are spending one of a very small number of repairs, and the second one goes worse than the first.

The second surgery is a different question

When a reconstructed ligament fails and a pitcher needs a revision, return to play falls to 711 in 1,000 from 798.

That still sounds survivable, until you notice the population has changed. A man having a second reconstruction has already shown you something: the first one did not survive what he asked of it. His odds cannot be read off the first-surgery number, because he is no longer in that group.

And here is the part nobody can tell you

So far we have counted pitchers who had the operation. Now try the question a worried parent actually asks: how likely is my kid to need it?

Mostly, nobody knows, and the reason is worth sitting with.

College baseball is the one level anyone has counted both halves of: 320 reconstructions among 27,959 Division I pitchers over seven seasons. That is about 11 in every 1,000. Warm-weather states run higher than cold ones, and that gap is real.

Pro baseball is the opposite. The surgeries are counted exactly — all 2,281 of them. The number of pitchers at risk is not published at all. You can know precisely how many operations happened and still not know anyone’s odds.

High school is worse, and this is the one to watch for. You will read that teenagers are 57% of all Tommy John surgeries in America. True. But the bottom of that fraction is the entire United States population, not the population of high school pitchers. It tells you who fills the waiting rooms. It tells you nothing about your son’s risk. It is quoted constantly as though it did.

Junior college? We looked. There is nothing.

What to take home: ask what counted as success

Strip the baseball away and this happens everywhere.

A drug is 90% effective. A school has a 95% graduation rate. A strategy beats the market 80% of the time. Before you decide whether any of that is impressive, ask two questions in order.

What counted as success? And who was counted at all?

The arithmetic tells you how often something happened. The definition tells you what happened. A percentage can be worked out perfectly and still answer the wrong question. When the bottom of the fraction is missing, it answers none at all.

So the next time somebody tells you Tommy John surgery has a 90% success rate, do not argue about the 90. Ask what the pitchers did.

Primary return to play 57–100%, same level 61–95%. High school and college: 73–85% and 55–74%. Revision: 67–98% and 42–78%. The width of those ranges is the argument of this piece, not a flaw in it.

On performance after return: both Hones et al. and Coughlin RP, et al. (“Return-to-Play and Competitive Outcomes After Ulnar Collateral Ligament Reconstruction Among Baseball Players: A Systematic Review,” Orthopaedic Journal of Sports Medicine 2021) report most studies finding reduced innings pitched after reconstruction, with several reporting decreased fastball velocity and mixed results on ERA. We could find no study reporting that a reconstructed ligament outperforms an uninjured one. The “comes back stronger” belief appears to be a cause-and-effect swap: a year of supervised strength work and mechanical rebuilding gets credited to the graft.

On definitions: Buchanan et al., “Current Definitions of Return to Play After Medial Ulnar Collateral Ligament Injuries and Surgery in Professional Baseball Players Prohibit Cross-Study Comparison: A Systematic Review,” Arthroscopy 2025. A separate review of 1,156 patients found no validated, universally accepted criteria for deciding when a pitcher is ready to return at all.

College incidence from Pescatore SM, DeShazo SJ, Weiss WM, “Frequency of Tommy John Surgery in NCAA Division I College Pitchers Versus Weather Conditions,” Orthopaedic Journal of Sports Medicine 2025: 320 reconstructions against 27,959 Division I pitchers, 2015–16 through 2021–22; warm states 0.0156 per pitcher against 0.0091 in cold states, P = .00011.

One limit we could not close, stated rather than hidden. We could not establish from the text available to us whether that 27,959 counts pitchers or pitcher-seasons. Division I carries roughly 5,000 pitchers a season, so across seven seasons the figure reads like seasons — which would make 11 in 1,000 a single-season risk, and a four-year career something closer to four times that. Same numerator, two reference classes, a four-fold difference in the answer. We have quoted only the per-season reading and no career figure.

The teenage share is from Erickson et al., who report 15- to 19-year-olds as 56.7% of US ulnar collateral ligament reconstructions performed between 2007 and 2011, growing about 9% a year against an overall national rate of 3.96 per 100,000 people. That is a share of patients against a general-population denominator, and it does not convert into a risk per player.
